Health Management and Support Programs

This program is designed to improve the wellbeing and management of patients diagnosed with Type 2 diabetes.

The program includes a customised plan with agreed goals provided with step-by-step clinical support. Care is provided in the comfort of your home by a registered nurse, nurse practitioner or qualified Diabetes educator, dependent on the level of care required.

This program is designed to mitigate the risks of falling and to improve confidence and balance in identified patients.

Support with balance and falls management is available through a customised plan delivered by a physiotherapist for eligible members.

Members suffering from chronic disease, complex health or mental health issues who require ongoing support can register for Health Coaching.

This service provides telephone-based information and support for self-management of these health conditions.

This program is for approved conditions or when medically necessary, at recognised providers, subject to eligibility requirements, which may include providing us with a letter from your medical practitioner.

Hospital at Home is a personalised service designed to make the transition from hospital to home quicker and easier for you.

If it’s appropriate, you may be able to leave hospital sooner after an operation and continue receiving the care and support you need in the comfort of your own home. By working with the hospital staff and your doctor, services including specialised nursing care and home help can be arranged to help you recover sooner at home.

In some situations, Hospital in the Home has helped members avoid re-hospitalisation.

How do I enrol?

All cases are individually reviewed by the hospital prior to your discharge to the care of your GP, with a care plan developed appropriate to your clinical needs.

Prehabilitation is a physiotherapist led exercise program that aims to improve your pre-surgery function. This in turn positively impacts your recovery and potentially reduces post-surgery complications.

How do I enrol?

Contact us to find out whether prehabilitation is included in your hospital cover. Provided that you are also clinically eligible, Health Partners can help to facilitate referral into the program through our partner YHN.

Rehabilitation at Home provides an alternative to hospital-based rehabilitation.

Instead of staying in hospital for rehabilitation, you get a tailored therapy program that can be provided in the comfort and privacy of your own home.

Your program, delivered by an experienced rehabilitation team, provides a short term therapy solution to help regain your independence and improve your quality of life.

How do I enrol?

Referral to the program is made by the staff of the hospital (this may be your doctor/specialist or discharge coordinator). This program is not available in all areas.

Chemotherapy at Home is a tailored program that provides home-based infusion therapy by highly trained and skilled nursing staff for specified cancer diagnoses and other inflammatory diseases.

Having ongoing infusion care in the comfort of your own home can provide eligible members with a convenient alternative to receiving treatment in hospital and reduced anxiety about associated travel costs, reliance on carers or child care, and exposure to infections.
