Dental

Health Partners Dental

At Health Partners Dental you receive:

• 85% benefit or discount for general dental services
• 85% benefit or discount for major dental services
• 70% benefit or discount for specialist treatment.

If you reach your limit at Health Partners Dental you receive a further unlimited 40% benefit.

Other Dental

A schedule of benefits applies for general, major and specialist dental services provided by dentists other than Health Partners professionals. Please call our Member Services for a quote.

Dental Limits

Orthodontic

A 70% benefit is available for all orthodontic treatment.

• Including appliances such as plates, guards and spacers
• Lifetime limit of $500 after 1 year, or
• Lifetime limit of $1,500 after 2 years, or
• Lifetime limit of $2,500 after 5 years.

Optical

Health Partners Optical

At Health Partners Optical you receive:

• 70% benefit or discount for prescription glasses
• 70% benefit or discount for contact lenses
• 30% saving on all sunglasses.

If you reach your limit at Health Partners Optical you receive a further unlimited 40% benefit.

Other Optical

At Other Optical you receive:

• 50% benefit for prescription glasses and contact lenses provided by optometrists other than Health Partners professionals.

Optical Limits

Benefits claimed at Health Partners Optical will count towards Other Optical limits, and vice versa.

Pharmacy

Health Partners Pharmacy

Exclusive pharmacy benefits are available at over 40 Health Partners participating pharmacies.

For an extensive range of prescriptions:

• Pay only $16.65 or $33.30
• Limit $600.

Your payment is indexed to the federal government PBS co-payment which is reviewed each year. Prices effective 1 January 2010.

Special member offer:

• You receive a 20% saving on non-prescription items such as vitamins, cosmetics, gifts, toiletries and more — no limit.

Physiotherapy

Health Partners Physiotherapy

At Health Partners Participating Physiotherapists you pay only a set fee for each visit.

• Initial consultation – pay only $12
• Subsequent consultation – pay only $12
• Long consultation – pay only $20
• Lymphoedema treatment – pay only $25
• Home or hospital visit – $ pay only $20
• Hydrotherapy – pay only $5
  Limit $500.

Other Physiotherapy

The following benefits are available for services provided by Physiotherapists other than Health Partners participating professionals.

• $19.50 subsequent consultation
• $24 long consultation
• $28 prolonged consultation
• $50 lymphoedema treatment
• $19 home or hospital visit

Total combined Physiotherapy limit is $1,000.

Naturopathy/Homoeopathy

Benefit:

• $26.40 initial consultation
• $20.40 subsequent consultation
• $28.80 long consultation
• $36 after-hours consultation
  Combined limit $350 for Naturopathy/Acupuncture.

Acupuncture

Benefit:

• $27.60 initial consultation
• $21 subsequent consultation
• $30.60 long consultation
• $37.80 after-hours consultation
  Combined limit $350 for Naturopathy/Acupuncture.

Chiropractic/Osteopathic

Benefit:

• $30 initial consultation
• $22 subsequent consultation
• $25 long consultation
• $29.50 after-hours consultation
• $30 home visit
• $35 after-hours home visit
• Radiology 60% of fee charged (in professional's rooms)
  Limit $350.

Podiatry

Benefit:

• $25 initial consultation
• $19 subsequent consultation
• 85% surgery
  Limit $350.

Orthotics

• A schedule of benefits applies for orthotics provided by approved orthotists, podiatrists and medical practitioners.
  Limit $300.

Dietary

Benefit:

• $60 initial consultation
• $35 subsequent consultation
• $15 group consultation
  Limit $450.

Massage

Benefit:

• $20 per consultation for remedial or therapeutic massage
  Limit $100.

Psychology

Benefit:

• $75 initial consultation
• $70 subsequent consultation
• $70 couple/family consultation per couple/family
• $20 group consultation
  Combined limit $400 for Psychology/Hpnotherapy.

Hypnotherapy

Benefit:

• $60 per consultation
  Combined limit $400 for Psychology/Hypnotherapy.

Speech Therapy

Benefit:

• $75 initial consultation
• $45 subsequent consultation
• $60 long consultation
• $15 group consultation
  Limit $400, $600 after 5 years of continual membership.

Eye Therapy

Benefit:

• $25 per consultation
  Limit $250.

Occupational Therapy

Benefit:

• $50 initial consultation
• $40 subsequent consultation
• $15 group consultation
  Limit $350, $500 after 3 years of continuous membership.

RDNS

Limit $150.

Full Ambulance Cover

As a Health Partners Extras member you receive full ambulance cover including:

• All emergency and non-emergency services
• Australia-wide coverage
• Road and air ambulance services
• Every service required on medical grounds (excludes clinic-car type transport)
  Limit $20,000.

Apparatus

You receive 85% of fee charged for the following apparatus:

• Asthmatic Spray Appliance — limit $130 every 3 years. Certain asthmatic spray appliances can be purchased through Health Partners for a minimal cost.
• Blood Glucose Monitoring Machines — limit $250 every 3 years. Health Partners members can purchase certain machines from the Diabetic Association of SA for a minimal cost including 12 months free membership to the Association.
• Hearing Aids — limits based on years of continuous membership: 1–4 years — $800; 5–9 years — $900; 10+ years — $1,150. Maximum benefit limited to once every 5 years.
• Sleep Apnoea Machines — limit $1,500 every 5 years.
• CPAP Apparatus Support — limit $150 every 5 years when using a Health Partners Participating Provider.

All apparatus benefits need a letter from your medical practitioner.

Health Management

• Online Health Profile — limit 4 per year. Access the Online Health Profile through Members Online.
• Quit Smoking — $100 benefit towards the cost of Nicotine Replacement Therapy when purchased at Health Partners Participating Pharmacies. Available on completion of a government-approved Quit Smoking program. Lifetime limit $200.
• Weight Watchers — $50 benefit for first month of membership, plus $100 benefit upon achieving goal weight. Lifetime limit $150.
• Bowel Cancer Screening — $22 benefit towards testing fees for a home Bowel Cancer Screening test. Limit 1 per year.
