Your Local Professionals

Dental

A schedule of benefits apply for general, major and specialist services. Please call our member services for a quote.

• General Dental – no limit
• Major dental – limit $1000
• Specialist treatment – limit is the same as general and major dental.

Orthodontic

70% benefit is available for all orthodontic treatment:

• Includes appliances such as plates, guards and spacers
• Lifetime limit of $500 is available after 1 year, or
• Lifetime limit of $1,500 is available after 2 years, or
• Lifetime limit of $2,500 is available after 5 years.

Optical

55% benefit for prescription glasses and contact lenses.

• Overall limit of $300 (this includes a limit for frames of $75).

Pharmacy

Pharmacy Connect

• You pay only $16.65 or $33.30 for an extensive range of prescriptions - limit $600 (includes your local Pharmacy prescriptions). Your payment is indexed to the Federal Government PBS and reviewed each year. Prices effective 1 January 2010.
• You receive a 20% saving on all non-prescription items and the convenience of ordering by email, phone or fax

• You pay $33.30 for each prescription not listed on the PBS - Limit $600 (includes Pharmacy Connect prescriptions). Prices effective 1 January 2010.

• $57 lymphoedema treatment
• $6.50 hydrotherapy

• Combined limit $500 for Naturopathy/Acupuncture.

Acupuncture

Benefit:

• $37.20 initial consultation
• $27.60 subsequent consultation

• Combined limit $500 for Naturopathy/Acupuncture.

Chiropractic/Osteopathic

Benefit:

• $34 initial consultation
• $27 subsequent consultation
• $32 long consultation

• Radiology 60% of fee charged (in professional's rooms)
• Limit $500.

Podiatry

Benefit:

• $27 initial consultation

Orthotics

Benefit:

• A schedule of benefits apply for orthotics provided by approved orthotists, podiatrists and medical practitioners
• Limit $300.

Dietary

Benefit:

• 60% per consultation with a registered dietitian
• Limit $250.

Psychology/Hypnotherapy

Benefit:

• 50% per consultation
• Limit $350.

Massage

Benefit:

• $20 per consultation for remedial or therapeutic massage
• Limit $100.

Speech Therapy

Benefit:

• $75 initial consultation
• $45 subsequent consultation
• $60 long consultation
• $15 group consultation
• Limit $400
• Limit $600 after 5 years of continuous membership.

Eye Therapy

Benefit:

• $25 per consultation
• Limit $250.

Occupational Therapy

Benefit:

• $50 initial consultation
• $40 subsequent consultation
• $15 group consultation
• Limit $350
• Limit $500 after 3 years of continuous membership.

Royal District Nursing Service

• Limit $150

Apparatus

You receive 85% of fee charged for the following apparatus:

• Asthmatic Spray Appliance — limit $100 every 3 years. Alternatively, certain asthmatic spray appliances can be purchased through Health Partners for a minimal cost.
• Blood Glucose Monitoring Machines — limit $250 every 3 years. Health Partners members can purchase certain machines from the Diabetic Association of SA for a minimal cost including 12 months free membership to the Association.
• Hearing Aids — limits based on years of continuous membership: 1–4 years — $800; 5–9 years — $900; 10+ years — $1,150. Maximum benefit limited to once every 5 years.
• Sleep Apnoea Machines — limit $1,500 every 5 years.

All apparatus benefits need a letter from your Medical Practitioner.

Health Management

• Online Health Profile — limit 4 per year. Access the Online Health Profile through Members Online.
• Quit Smoking — $100 benefit towards the cost of Nicotine Relacement Therapy when purchased through Pharmacy Connect or your local pharmacy. Available on completion of an approved government Quit Smoking program. Lifetime limit $200.
• Weight Watchers — $50 benefit for first month of membership, plus $100 benefit upon achieving goal weight. Lifetime limit $150.
• Bowel Cancer Screening — $22 benefit towards testing fees for a home Bowel Cancer Screening test. Limit 1 per year.

Full Ambulance Cover

As a Health Partners Extras member you receive full ambulance cover including:

• All emergency and non-emergency services
• Australia-wide coverage
• Road and air ambulance services
• Every service required on medical grounds (excludes clinic-car type transport)
• Limit $20,000.
